Facility's tank form is not current. The overfill prevention information is not correct. New form was <br />provided and the owner updated the information. A new facility form was also filled out. <br />4. Facility does not have a current financial responsibility certification. This is a violation of Health and <br />Safety Code 25292.2. A new financial responsibility certification was provided and the owner updated <br />the information. This is a repeat violation. This is a annual requirement. evil -'e- <br />11. The liquid sensor in the 91 turbine sump was not located at the lowest point. The 87 annular sensor <br />is 7 inches short thus not located at the lowest point. Pursuant to California Code of Regulation section <br />2630(d) the sensors will not detect a leak at the earliest opportunity. A new sensor (model number <br />794390-409, serial number 799144) was installed and was placed jp the correct location. A permit must <br />be obtain from the EHD by the next business day. Ivw <br />12. Liquid sensor at dispenser number 1/ 2 failed the hyd ostatic test. Pursuant to H&S code 25291(b) <br />a facility must have a monitoring system capable of detecting a leak from the primary to the secondary <br />containment. A new sensor was installed ( Model number 794380-322, serial number 163361). Service <br />Technician Lupe Sanchez, ICC # 5250451 - UT. A permit must be obtained from this office by the next s <br />15. A secondary containment testing was performed on November 25. 2008, and the Regular fill sump <br />failed the test. A follow up letter for the repair and retest was sent on March 6, 2009 and since then no <br />action has been taken. Secondary containment for the UST system is not tight. Immediately, have the <br />UST system investigated, repaired and retested by a properly licensed trained and certified contractor <br />for the secondary containment testing. A permit may be required from the Environmental Health <br />Department. <br />24, 25; 26. Last monitoring certification leak detector test and the spill bucket test for the facility was <br />performed on November 25, 2008. Facility is 19 months late for a monitoring certification, leak detector <br />test and spill bucket test. This is a annual requirement. A monitoring certification leak detector test and <br />spill bucket test was performed today.
