(CLASS I CLASS it or MINOR Notice to Comply) <br /> ate....:. : c....a ... <br /> Item# Remarks <br /> 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator(DO)failed to document all the alarms from the attached alarm history on the May 2017 <br /> and September 2017 designated operator monthly inspection report. <br /> On the 5-12-2017 DO inspection report, David Winkler(DO),failed to note an L6 Dispenser 11-12 high liquid alarm <br /> that occurred on 5-11-2017. He noted the alarm and made sure it was responded to appropriately on the 6-30-2017 <br /> DO report. <br /> On the 9-19-2017 DO inspection report, Edward Stearns(DO)failed to note a L9 91 STP sensor out alarm, a L10 <br /> 91 annular fuel alarm, a L11 91 fill sensor out alarm, a L12 87 stp sensor out alarm, a L13 annular space sensor out <br /> alarm, a L14 87 fill sensor out alarm, a L15 diesel stp sensor out alarm and a L16 diesel fill sensor out alarm on <br /> 8/21/2017. The date the alarm history was printed out was also missing from the 9-19-2017 DO inspection report. <br /> The facility did document the alarms nd responded to them appropriately. <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Approximately 1 gallon of liquid was observed in the87 grade fill sump. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this <br /> liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. <br /> Ensure that the 87 grade fill sump is maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> 318 CCR 2636(f)(2)Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87 grade VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br /> detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it <br /> passed. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635(b) No spill bucket,failed to contain five gallons, or failed to provide a means to keep it empty. <br /> The 91 grade spill container failed to have a minimum capacity of five gallons. Dripping was observed during the <br /> spill container test. All spill containers shall have a minimum capacity of five gallons. The technician lowered the <br /> water amount and determined that too much water was added to the spill container. When the spill container was <br /> retested,there was no drop observed(water level was measured). Slight driping was still observed. The service <br /> technician said it was the left over water from when the first test occurred and declared the bucket a pass. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> FA0012355 PRO515864 SCO02 12/28/2017 <br /> EHD 23-01 Rev.09/22/16 Page 5 of 6 Underground Storage Tank Program OIR <br />
