.. t Midi y <br />The undersigned, <br />whose address is <br />represents and certifies as follows: <br />I. The undersigned is the tenant ("Tenant") under that certain lease ("Lease") <br />dated <br />Landlord, covering <br />with <br />the premises commonly known as <br />("Demised Premises"). <br />11. The Lease constitutes the only agreement (either written or oral) the undersigned has <br />with respect to the Demised Premises and any right of occupancy or use thereof. <br />Ill. The Lease is in full force and effect and has not been assigned, subleased, <br />supplemented, modified or amended except as follows: <br />as <br />IV. The undersigned presently occupies the Demised Premises and is paying rent on <br />a current basis. <br />No rent has been paid by Tenant in advance except for the monthly rental that became <br />due on <br />the Lease. <br />V. The monthly rental is the sum of <br />VI. The present Lease term expires on <br />no options to renew except: <br />, and a security deposit in the sum of now held by Landlord in accordance with <br />the terms of <br />Dollars <br />and there are <br />VII. There are no defaults under the Lease by Landlord or any events which with the <br />passage of time or giving of notice or both will result in any such default.
