Contact Fire Department for permit. <br />2. All new submittals supercede the previous ones. <br />3. Automatic pump -shutdown must satisfy all conditions listed in CCR 2636(g). <br />4. Overfill prevention device, OPW 61 SO -410C, shall be installed at 90% of tank <br />volume in order to achieve a total shut -down at 95%. <br />5. Submit certification of financial responsibility and letter from chief financial officer <br />to EHD prior to scheduling tank setting appointment. <br />6. Contact Michael Kith for inspection. See New UST Installation Inspection Record <br />for the required inspection. All appointments must be scheduled at least 48 hours in <br />advance. <br />7. Please be informed that SB 989 may require secondary containment on all vapor <br />return lines in future. <br />8.
