This work was completed <br /> on July 05, 2017. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to commencing <br /> work. All new addenda supersede the previous ones. <br /> 3. Contact Elianna Florido at(209)468-0343 to arrange the following appointments (a 48-hour notice is <br /> required for inspection appointments). Please be aware that inspections for emergency work must be <br /> scheduled within five days of permit approval. <br /> • Leak detector test:The leak detector was tested on 7/5/2017 and was confirmed to detect a leak of 3 <br /> gallons per hour at 10 psi. No further inspections are required by the EHD at this time. <br /> 4. If concrete will be broken near diesel tank, a secondary containment test must be performed on the diesel <br /> tank once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, current contractor's <br /> licenses issued by the Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically present at the work <br /> site who possesses a valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s)of the component(s) being tested, <br /> repaired, or serviced. <br /> 1868 E.
