Hammer Lane, Stockton, CA 95205 SR0074032 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to disassemble the diesel STP sump turbine product line flex <br />connector, clean and reapply pipe sealant on the underground storage tank (UST) system at the <br />above noted address. The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Fatinah Zareef at (209) 953-7311 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Primary piping line tightness test: A primary piping line tightness test must be performed <br />by a SWRQB licensed Tank Tester and witnessed by the EHD. <br />Any individual performing the work of a service technician must meet the following <br />qualifications: <br />b. Possess or be employed by a person who possesses one of several permissible <br />valid, current contractor's licenses issued by the Contractors State License Board <br />(may be issued to the installation company), or a Tank Tester's license issued to the <br />individual by the State Water Board, and <br />c. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service <br />Technician certificate issued by the International Code Council (ICC), and <br />d. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />
