Address: 3304 W Hammer Lane, Stockton I Date: Jun 21, 2012 <br />SUMMARY OF VIOLATIONS <br />(CLASS 1, CLASS 11, or MINOR -Notice to Comply) <br />Routine and Monitoring System Certification LIST Inspection <br />108. Secondary containment testing was last performed on 12-23-2008 and was due by December <br />2011. Testing has not been completed and is six months past due. Secondary containment testing is <br />required once every 36 months. Immediately schedule this test and provide 48 hours notification to the <br />San Joaquin County Environmental Health Department (EHD). <br />203, 305, 306. Dispenser 1/2 had approximately a one half inch deep collection of diesel in the <br />dispenser sump today. The dispenser piping has a small leak. The technician adjusted the pipe fittings <br />to eliminate the leak. The float and chain mechanism did not activate from the leak and the technician <br />adjusted it to work correctly. <br />Any release from the primary containment that does not escape the secondary containment and was <br />cleaned up within eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br />primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's <br />monitoring records. Submit a copy of the recordable release statement to the EHD by July 21, 2012. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHDS CURRENT HOURLY RATE. <br />EHD 23-02 Rev 04/19/12 Page 3 UST INSPECTION REPORT <br />
