The permit has been <br />approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br />backfilling. <br />b. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />EHD inspection and approval. <br />4. Since concrete will be broken near the Diesel tank, a secondary containment test must <br />be performed on the Diesel LIST once concrete is poured in accordance with the <br />manufacturer's guidelines. Send the test results to the EHD. <br />5. A "UST Certification of Installation/Modification" form shall be submitted by the owner or <br />the owner's agent (CCR Section 2635(d)) before this upgrade is considered complete by <br />EHD. <br />6. All components must be certified by an independent testing organization and labeled as <br />required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br />Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br />
