You were required to send this information to the EHD by July 22, 2009. A <br />complete set of information has not been received by the EHD. <br />Provide the following: <br />Current UST Facility form (former Form A) <br />r Annual Monitoring Certification <br />Monitoring and Response plans <br />Certification of Financial Responsibility and <br />Letter from Chief Financial Officer <br />r Designated Operator employee training <br />records <br />Certification of Return to Compliance (Signed <br />and dated) <br />Recordable release statement for diesel leak <br />under dispenser #1. <br />17- Current UST Tank form (former Form B) <br />I— Annual monitoring certification, leak detector <br />and spill bucket test results <br />(- Maintenance and monitoring logs <br />F Designated Operator reports <br />F Statement of Designated Operator <br />Statement of how each violation was or will <br />be corrected <br />r <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />department rate, currently $115/hour. Failure to submit the requested information immediately may result <br />in a reinspection or further legal action. <br />Forms and the inspection report are provided with this letter.
