The following is an itemized list of underground storage tank violations that have not <br /> been addressed for HAMMER LANE OIL INC as of August21, 2017. <br /> Open violations from June 12, 2017 inspection <br /> Violation#103-Current financial responsibility documents not submitted. <br /> Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents <br /> are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, and upload the required documents. <br /> Violation #107 -Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The site map is missing the locations of the leak <br /> detectors for each tank. A site plan must be submitted identifying the locations where monitoring will be performed. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload <br /> a copy of the UST Monitoring Site Plan. <br /> Violation#205 -Failure to have an approved UST response plan available onsite. <br /> An approved copy of the response plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> Violation#206 -Failure to comply with UST operating permit conditions. <br /> The UST system possibly had a cold start on 12/09/2016 and a permit was not obtained from the EHD. According to <br /> the store associate, the UST system had some problems and mentioned it was taken care of by Elite IV. No work <br /> order or maintenance records were found. Emergency repairs relating to UST system monitoring and release <br /> detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy <br /> and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and <br /> approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the cold start. A <br /> "Failure to Obtain a Permit' penalty applies. <br /> Violation #209 -Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> The designated operator failed to document the follow up action for the alarms on the July 2016 and December <br /> 2016 designated operator monthly inspection reports and failed to check that they were responded to appropriately. <br /> The DO documented the alarms on June 24th, 2016, and December 9, 2016, but did not show if they were <br /> responded to appropriately. During the monthly inspection, the designated operator shall review the alarm history <br /> for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy <br /> of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> Violation #313 -Failure to construct, operate, and maintain primary containment as product-tight. <br /> Seepage was observed in the diesel piping in dispenser 1/2 above the sheer valve, indicating a leak in the primary <br /> piping. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, <br /> and certified contractor repair or replace the failed component under permit and inspection of the EHD. <br /> Open violations from June 13, 2016 inspection <br /> Violation#103 -Current financial responsibility documents not submitted. <br /> Financial responsibility documents have not been submitted to the EHD.
