The following is an itemized list of hazardous waste violations that have not been <br /> addressed for DOLLAR GENERAL# 15928 as of May 29, 2020. <br /> Open violations from August 09, 2019 inspection <br /> Violation#106-Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> 1. When asked how employees determine which of the differently categorized hazardous waste(HW)totes a waste <br /> item belongs in, Sue Dean,Assistant Store Manager, said each item is scanned using a hand-held scanner,which <br /> will sometimes state which HW tote the item belongs in. Sue indicated that when the scanner does not specify <br /> which HW tote an item belongs in, employees will read the item's label and ingredients, as well as the Dollar <br /> General (DG) HW guidance poster and documents,which provide product examples and key words for each of the <br /> HW categories,to determine which HW tote an item belongs in (see photos IMG 8084, 8094, 8098, 8100, 8102, <br /> 8104, 8107).When asked what employees do if they are still unsure about which HW tote a waste item belongs in, <br /> Sue stated she will ask her store manager, and if needed, her district manager or she will call the ERC/DG <br /> Corporate phone number for help. Sue was not aware of DG's Respond ticketing system to submit a notification to <br /> DG Corporate regarding the scanning system not providing a HW category for a particular waste item. <br /> 2.A GE Incandescent/Halogen Crystal Clear light bulb was found in a Universal Waste tote, however according to <br /> the DG Environmental Compliance HW poster and guidance documents, light bulbs(except for compact <br /> fluorescent bulbs and LED bulbs)are to be placed in the Toxic HW tote. (IMG 8084, 8090-8093) <br /> 3.An expired box of DG Health Coated Nicotine Gum,with an expiration date of 02/2019,was found for sale on a <br /> shelf in the facility. Sue removed the expired nicotine gum from the sales shelf and placed it into one of two open <br /> unlabeled baskets behind the front register. Sue stated that damaged items are placed into these two baskets <br /> behind the register and are processed once a week on Tuesday. Sue indicated that once a damaged item is <br /> processed it would be placed in a HW tote if needed. (IMG 8141-43) <br /> 4.A Satin Crush Lipstick was found in one of the open unlabeled basket behind the front register. Sue stated the <br /> basket is used to hold damaged items until they are processed every Tuesday. Sue stated that because the lipstick <br /> appeared to be opened, the lipstick would be damaged out and eventually placed in a HW tote. (IMG 8142, <br /> 8144-5) <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper <br /> waste handling and emergency procedures. <br /> Provide proof of re-training and training materials to the EHD for employees whose responsibilities include <br /> hazardous waste. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 1 <br />
