Geosyntec'= <br /> 4 PRE-FIELD ACTIVITIES <br /> This section describes the permitting, coordination, and health and safety tasks that will be <br /> conducted prior to the start of field activities. <br /> The following activities will be conducted prior to field activities: <br /> • Well installation permits will be obtained from San Joaquin County Environmental <br /> Health Department; <br /> • GSH will discuss work hours, and schedule with Simplot in advance of field <br /> implementation and well installations will be coordinated with them. <br /> • The site-specific health and safety plan (HASP)and task hazard analyses (THA)will be <br /> updated. The HASP will be available on-site during all field activities. Prior to the start of <br /> field activities and at the beginning of each work day, a safety tailgate meeting will be <br /> conducted; <br /> • All contractors working on Simplot property will successfully complete GSH's and <br /> Simplot's Contractor Health and Safety Training. <br /> • The well locations will be marked with white paint or stakes and Underground Service <br /> Alert(USA)will be contacted a minimum of 48 hours prior to commencement of drilling <br /> activities; and <br /> • A private utility locator will perform physical surveys to clear potential subsurface <br /> utilities, pipelines, or other obstructions at the proposed well locations prior to drilling <br /> work. <br /> Extraction Well Installation Work Plan 4 November 30,2018 <br />
