(GSH) Geosyntec is planning to install two monitoring wells <br /> (PW-35-280 and PW-36-280)at 2050 E Louise Avenue(APN 198-160-020).The selected driller, Yellow <br /> Jacket Drilling, is a C-57 certified environmental driller. Please find the enclosed well permit application <br /> package. Included in this application package are the following: <br /> • Attachment 1 - Well Permit Application for Approximately 17999 McKinley Ave; <br /> • Attachment 2—Site Characterization Work Plan Second Addendum; <br /> • Attachment 3 -RWQCB Approval for the work plan addendum; <br /> • Attachment 4—Well installation location; <br /> • Attachment 5—Well construction detail table; <br /> • Attachment 6—Well construction diagram; <br /> • Attachment 7—GSH and TEC Access Agreement; <br /> • Attachment 8—Yellow Jacket Drilling C-57 license and COI; and <br /> • Payment check in the amount of $710.00 ($456 for the CVRWQCB Oversight Work Plan <br /> Review fee,$304 for 2 monitor wells at 2050 E Louise Avenue). <br /> This work is scheduled to start on approximately January 2, 2019 and should be completed in <br /> approximately three weeks. The well will be installed per the Department of Water Resources Well <br /> Bulletin 74-81 and 74-90.
