(GSH) Geosyntec is planning to install one <br /> monitoring well (PW-46) at 16888 McKinley Ave, Lathrop (APN #19816026). The selected <br /> driller, Yellow Jacket Drilling is a C-57 certified environmental driller. Please find the enclosed <br /> well permit application package. Included in this application package are the following: <br /> • Attachment 1 - Well Permit Applications for 16888 McKinley Ave; <br /> • Attachment 2 —The Site Characterization Work Plan; <br /> • Attachment 3 - RWQCB Approval Email for the work plan; <br /> • Attachment 4— Well installation location; <br /> • Attachment 5 — Well construction detail table; <br /> • Attachment 6—Well construction diagram; <br /> • Attachment 7—Access Agreement; and <br /> • Payment check in the amount of $608.00 ($456 for the CVRWQCB Oversight Work <br /> Plan Review fee, $152 for 1 monitor well at 16888 McKinley Ave. <br /> This work is scheduled to start on November 19 2018 and should be completed in four weeks. <br /> The well will be installed per the Department of Water Resources Well Bulletin 74-81 and 74-90. <br /> The access agreement will be updated prior to the start of drilling activities on the Super Store <br /> facility at 16888 McKinley Ave. Please don't hesitate to contact me if you have any questions <br /> regarding this permit application.
