(GSH) Geosyntec is planning to install two <br /> monitoring wells one (PW-39) at 1909 E Louise Ave., Lathrop (APN #198100100), and the other <br /> (PW-40) at approximately 16353 McKinley Ave. in the City of Lathrop Right-of-way. The <br /> selected driller, Yellow Jacket Drilling is a C-57 certified environmental driller. Please find the <br /> enclosed well permit application package. Included in this application package are the following: <br /> • Attachment 1 - Well Permit Applications for 1909 E Louise Ave and Approximately <br /> 16353 McKinley Ave; <br /> • Attachment 2—The Site Characterization Work Plan; <br /> • Attachment 3 - RWQCB Approval for the work plan; <br /> • Attachment 4—Well installation location; <br /> • Attachment 5 —Well construction detail table; <br /> • Attachment 6—Well construction diagram; <br /> • Attachment 7—Access Agreement; <br /> • Attachment 8 —Approved Encroachment Permit; <br /> • Attachment 9— YellowJacket Drilling COI and C-57 License; and <br /> • Payment check in the amount of $760.00 ($456 for the CVRWQCB Oversight Work <br /> Plan Review fee, $152 for 1 monitor well at 1909 E Louise Ave and $152 for 1 monitor <br /> well at approximately 16353 McKinley Ave. <br /> This work is scheduled to start on November 19 2018 and should be completed in four weeks. <br /> The well will be installed per the Department of Water Resources Well Bulletin 74-81 and 74-90. <br /> The access agreement will be updated prior to the start of drilling activities on the 342 Sycanda <br /> LLC property at 1909 E Louise Ave. Please don't hesitate to contact me if you have any <br /> questions regarding this permit application.
