(GSH) Geosyntec is planning to install nine air-sparge <br /> wells in Lathrop, CA. Six wells (AS-4, AS-5, AS-6, AS-7, AS-8, AS-9) are planned at 16888 <br /> McKinley Ave (APN #19816026). Three wells (AS-1, AS-2, AS-3) are planned at 16777 <br /> Howland Road (APN#19818008). The selected driller, Yellow Jacket Drilling is a C-57 certified <br /> environmental driller. Please find the enclosed well permit application package. Included in this <br /> application package are the following: <br /> • Attachment 1 - Well Permit Applications for 16888 McKinley Ave and 16777 Howland <br /> Road; <br /> • Attachment 2 — The In Situ Bioremediation Pilot Test Work Plan submitted on 20 <br /> September 2018; <br /> • Attachment 3 - RWQCB Approval Email for In Situ Bioremediation Pilot Test Work <br /> Plan approving the work to begin; <br /> • Attachment 4—Well installation locations; <br /> • Attachment 5 —Well construction detail table; <br /> • Attachment 6—Well construction diagram; <br /> • Attachment 7—Access Agreement; <br /> • Attachment 8 —Yellowjacket C-57 license and COI; and <br /> • Payment check in the amount of$1,672.00 ($456 for the CVRWQCB Oversight Work <br /> Plan Review fee, $760 for 6 air sparge wells at 16888 McKinley Ave and $456 for 3 air <br /> sparge wells at 16777 Howland Road). <br /> This work is scheduled to start on November 12 2018 and should be completed in eight weeks. <br /> The wells will be installed per the Department of Water Resources Well Bulletin 74-81 and 74- <br /> 90.
