(GSH) Geosyntec is planning to install a groundwater <br /> monitoring well in Lathrop, CA. The monitoring well (PW-45) is planned to be drilled and <br /> installed at location 5B on the attached figure at 16888 McKinley Ave (APN #19816026). The <br /> selected driller, Yellowjacket Drilling is a C-57 certified environmental driller. Please find the <br /> enclosed well permit application package. Included in this application package are the following: <br /> • Attachment 1 - Well Permit Application for 16888 McKinley Ave; <br /> • Attachment 2—The Site Characterization Work Plan submitted on 22 June 2018; <br /> • Attachment 3 - RWQCB Approval Email for Site Characterization Work Plan approving <br /> the work to begin; <br /> • Attachment 4—Well installation location; <br /> • Attachment 5 —Well construction detail table; <br /> • Attachment 6— Well construction diagram; <br /> • Attachment 7—Access Agreement; <br /> • Attachment 8 —Yellowjacket C-57 license and COI; and <br /> • Payment check in the amount of $608.00 ($456 for the CVRWQCB Oversight Work <br /> Plan Review fee, $152 for one monitoring well at 16888 McKinley Ave). <br /> This work is scheduled to start on November 19 2018 and should be completed in two weeks. <br /> The wells will be installed per the Department of Water Resources Well Bulletin 74-81 and 74- <br /> 90.
