(GSH) Geosyntec is planning to install eight <br /> groundwater monitoring wells in Lathrop, CA. Six wells (PW-37, PW-38, PW-39, PW-40, PW- <br /> 41, and PW-42) are planned at 16888 McKinley Ave (APN #19816026). Two wells (PW-43 and <br /> PW-44) are planned at 16777 Howland Road (APN#19818008). The selected driller, Gregg <br /> Drilling is a C-57 certified environmental driller. Please find the enclosed well permit application <br /> package. Included in this application package are the following: <br /> • Attachment 1 - Well Permit Applications for 16888 McKinley Ave and 16777 Howland <br /> Road; <br /> • Attachment 2—The Site Characterization Work Plan submitted on 22 June 2018; <br /> • Attachment 3 - RWQCB Approval Email for Site Characterization Work Plan approving <br /> the work to begin (Attachment 4); <br /> • Attachment 4—Well installation locations; <br /> • Attachment 5 —Well construction detail table; <br /> • Attachment 6—Well construction diagram; <br /> • Attachment 7—Access Agreement; and <br /> • Payment check in the amount of$1,520.00 ($456 for the CVRWQCB Oversight Work <br /> Plan Review fee, $760 for 6 monitor wells at 16888 McKinley Ave and $304 for 2 <br /> monitor wells at 16777 Howland Road). <br /> This work is scheduled to start on July 23 2018 and should be completed in twelve weeks. The <br /> wells will be installed per the Department of Water Resources Well Bulletin 74-81 and 74-90. <br /> The access agreement will be updated prior to the start of drilling activities on the Super Store <br /> facility at 16888 McKinley Ave. Please don't hesitate to contact me if you have any questions <br /> regarding this permit application.
