(GSH) Geosyntec is planning to install one new <br /> monitoring well (PW-47) in the City of Lathrop property, located near 17550 Shideler Parkway <br /> in Lathrop, CA. The selected driller, Gregg Drilling, LLC is a C-57 certified environmental <br /> driller. Please find the enclosed well permit application package. Included in this application <br /> package are the following: <br /> • Attachment 1 - Well Permit Application for PW-47 <br /> • Attachment 2 — The Site Characterization Work Plan and associated addendum for PW- <br /> 47; <br /> • Attachment 3 - RWQCB Approval for the work plan; <br /> • Attachment 4—Well installation location map; <br /> • Attachment 5 —Well construction detail table; <br /> • Attachment 6—Well construction diagram; <br /> • Attachment 7—Approved Encroachment Permit; <br /> • Attachment 8 —Gregg Drilling LLC, COI and C-57 License; and <br /> • Payment check in the amount of$608 ($456 for the Work Plan Review fee, and $152 for <br /> 1 monitor well in the City of Lathrop ROW, located at approximately 16051 to 16407 <br /> McKinley Avenue. <br /> This work is scheduled to start on May 13, 2019 and should be completed in three weeks.
