TADDEI ROAD <br /> ACAMPO, CALIFORNIA <br /> 10/03/89 Lodi Concrete Pipe Service submitted to Public Health <br /> Services, Environmental Health Division (PHS/EHD) an <br /> Underground Storage Tank (UST) Removal Application for <br /> two (2) tanks; Tank #1/500 gallon leaded gasoline and <br /> Tank #2/350 gallon unleaded gasoline. <br /> 10/25/89 PHS/EHD issued UST Removal Permit for two (2) gasoline <br /> tanks. <br /> 10/31/89 Two (2) UST were removed under permit and inspection. <br /> Inspector notes slight petroleum odor in soil under tank <br /> #1. Two (2) soil samples were collected for analyses. <br /> 11/20/89 Soil reports were received, both samples tested positive <br /> for gasoline. A prop. 65 and UAR were completed. <br /> 11/27/89 PHS/EHD contacted Mrs. Lorz, the responsible party (RP) , <br /> and explained to her that the soil sample results <br /> indicate contamination present, <br /> 12/06/89 Upon receiving the Contaminated Soil Work Plan, this site <br /> was selected for placement into the Local Oversight <br /> Program (LOP) , formerly the Pilot Program. <br /> 12/12/89 Written correspondence was prepared and sent to Lodi <br /> Concrete Pipe Service which notified the RP that the site <br /> was officially assigned to the LOP. <br /> 05/14/90 This office received a letter from Ron and Alison Conway <br /> who had recently purchased the property stating, "We are <br /> not assuming responsibility for the cleanup. " <br /> 05/23/90 PHS/EHD prepared and sent a letter to the RP instructing <br /> them to submit a Soil Contamination Work Plan and <br /> Groundwater Investigation Proposal by July 15, 1990. <br /> 07/16/90 A Soil Remediation Work Plan and Soil Remediation <br /> Proposal was received and reviewed. <br /> 07/18/90 Over-excavation of the tank pit was performed during <br /> inspection by this office and five (5) soil samples were <br /> collected in the final excavation and petroleum odors <br /> were noted in two (2) samples. <br />
