The Addendum indicated that the original scope of work
proposed in the work plan dated April 4,1994 which included the drilling of one slant and two
vertical soil borings, could not be performed because the excavation had been backfilled from 17
to 2 feet below grade, preventing access by a drill rig.
The work proposed in the original work plan was performed, with modification, without PHS/EHD's
prior notification and hence was not recognized as valid. State Water Resources Control Board
SWRCB) Underground Storage Tank Cleanup Fund staff has indicated that the work performed, not
recognized as valid, will not be eligible for reimbursement.
The Addendum's proposed scope of work also should provide the information necessary to evaluate
the residual soil contamination. The Addendum indicated that up to three soil samples will be
collected from some soil borings, while the original work plan only proposed that two would be
collected. An estimate of the costs associated with the Addendum should be submitted to the
SWRCB Cleanup Fund Unit staff for evaluation.
As PHS/EHD stated in correspondence dated May 4, 1994, please provide PHS/EHD with 48 hours
notice prior to initiating field activities so that PHS/EHD can witness soil sampling and grouting.
Donna Heran, REHS Director
Environmental Health Division
Mary Meays, Senior REHS Diane M. nson, REHS
Site Mitigation Unit Supervisor
MM13049H 12.D
cc: Elizabeth Thayer, CVRWQCB
cc: George Lockwood, SWRCB Cleanup Fund Unit
cc: Marc Crum, Condor Earth Technologies
cc: Keith Tallia, Oil Equipment Service
A Division of San Joaquin Cnunlr Health Cure Services
