Tenth Str. <br /> Tracy CA 95376 <br /> San Joaquin County _?sblic Health Services Environmental Health <br /> Division (PHS/EHD) has recently reviewed documents pertaining to <br /> the above mentioned site. You were notified by PHS/RHD in <br /> correspondence dated March 8 , 1994 that further investigation of <br /> the extent of contamination from the former underground storage <br /> tanks must be conducted. After analysis of PHS/EHD inspection <br /> reports, laboratory data, and shallow depth to groundwater, this <br /> site has been placed in PHS/EHD' s Underground Storage Tank Local <br /> Oversight Program for the oversight of assessment and clean-up <br /> activities . <br /> The attached "Notice of Reimbursement" is official notification <br /> that PHS/EHD' s Underground Storage Tank Local Oversight Program <br /> has identified you as a responsible party for the above mentioned <br /> site. Fees for review activities and inspections associated with <br /> contaminated site assessment activities will not be billed by this <br /> office . The State Water Resources Control Board will invoice you <br /> directly for activities performed under the auspices of the Local <br /> Oversight Program. The goal of the site evaluation performed by <br /> PHS/EHD is to certify closures of underground tank leakage sites . <br /> Closure certification occurs only after mitigation activities are <br /> completed. <br /> In addition, The "Regional Board Staff Reommmendations For Initial. <br /> Evaluation and Investigation of the Underground Tanks'- and <br /> "Appendix All reporting format and the LUFT Manual shall be Utilized <br /> as guidance documents in the preparation of work plans and/or <br /> reports . <br /> A work plan to initiate soil investigation should be submitted <br /> within 60 days of the date of this letter.
