October 31, 1997 <br /> Additional Conditions of Encroachment Permit No 97-198 <br /> 1 The Permittee shall submit the results of samples analysis to the City for the soil <br /> borings taken within Hamilton Alley <br /> 2 The Permittee shalltY rovide the Ci copy of access agreement from adjacent <br /> P <br /> property owner(s) for City records <br /> 3 The Permittee shall fill soil boring voids with concrete mortar The top 7 inches shall <br /> be replaced with asphalt concrete cold mix <br /> 4 The Permittee shall submit a letter to the City assuring that the responsible Agency <br /> will perform at his own cost the remediation and clean-up work of contaminated soil <br /> and groundwater caused by discharge from the San Joaquin County property The <br /> installation of extracting and monitoring wells that might be needed is not covered <br /> under this permit <br /> 5 In the event soil and groundwater contamination is found within Hamilton Alley after <br /> the completion of the samples analysis, the Permittee shall inform the City of the <br /> schedule of remediation and clean-up work by the responsible Agency <br /> 6 The Permittee shall provide the City a copy of the soil boring permit issued and <br /> approved by San Joaquin County Environmental Health Division <br /> 7 The Permittee shall maintain access to and from properties adjacent the <br /> construction site at all times <br /> 8 The Permittee shall coordinate the soil boring work with PG&E, Pacific Bell, TCI and <br /> other utility owners that may have underground facilities on the construction site <br /> Any damage to existing facility(s) or improvements, whether private or public <br /> improvements, shall be restored or replaced to it's original or better condition at the <br /> Permittee's cost and at the satisfaction of the City <br /> Prepared by <br /> ri <br /> C seldo S Mina, P E <br /> Associate Civil Engineer <br /> cc Nanda Gottiparthy, Senior Civil Engineer <br /> 05-110497sd <br />.s <br />
