Memo <br /> To: Jamie Lima, Environmental Health Department <br /> From: im Spicer, Little Potato Slough Mutual Water Company <br /> Subject: rrective action statement <br /> Date: August 31, 2016 <br /> In response to the Hazardous Materials Program Inspection Report dated August 19, 2016 we have <br /> taken the following actions to correct the violation "Failed to provide or document initial and/or <br /> refresher training to appropriate personnel": <br /> 1. Prepared and distributed a training manual covering all hazardous chemicals in the CERS <br /> inventory for LPSMWC. <br /> 2. Reviewed the training manual material with each employee. <br /> 3. Created record files for each employee and stored signed acknowledgement of training from <br /> each employee. <br /> 4. Created a record file for the training manual. <br /> 5. Created a record file for the Emergency Response/Contingency Plan. <br /> All file records are stored in the Clarifier Plant office. <br />
