S0 JOA U 1 Environmental Health Department
U ; i y I AMENDED
3/9/2020
Small Quantity Hazardous Waste Generator Inspection Report
Facility Name: Facility Address: Date:
Banfield Pet Hospital#1188 10520 Trinity Pkwy, Stockton November 20, 2019
SUMMARY OF VIOLATIONS
(CLASS I,CLASS II,or MINOR-Notice to Comply)
Item# Remarks
words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition
(contents), accumulation start date
CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective
action statement and supporting documentation to the San Joaquin County Environmental Health Department
(EHD)within 30 days.
This is a Class II violation.
Overall Inspection Comments:
Notes:
-The facility hazardous waste generator has been changed back to Small Quantity generator status from RCRA
Large Quantity generator status, since the business operator provided a written statement to the EHD, as well as
a manifest correction letter to the DTSC, explaining the facility's epinephrine waste was mistakenly listed as
P042 (a RCRA acute hazardous waste). The operator clarified that the epinephrine waste consists of an
epinephrine salt, which is not included within the scope of the P042 listing, according to the EPA. Moving
forward, the operator is to ensure the correct waste codes are listed on the facility's hazardous waste manifests.
-The inspection report has been amended by changing the violations to reflect the facility's Small Quantity
Generator status, including violation 108.
-According to the Banfield Pet Hospital Waste Management Plan, Banfield disposes of all non-controlled
pharmaceutical waste, and only the controlled pharmaceutical waste listed on Appendix A of the plan, as
hazardous waste. Any other controlled pharmaceutical waste, not listed on Appendix A, are managed in
accordance with Banfield's Returning and Expired Drugs policy. Please submit a statement to the EHD
explaining if Banfield has made a determination as to which pharmaceutical wastes are RCRA vs non-RCRA, for
both the non-controlled and the controlled pharmaceutical wastes.
A Small Quantity Hazardous Waste Generator inspection checklist was provided to the facility operator on the
day of inspection. The EHD amended and wrote a RCRA Large Quantity Hazardous Waste Generator report on
11/26/2019 which replaced the initial checklist. The EHD amended and wrote a Small Quantity Hazardous
Waste Generator report on 3/9/2020, which replaces the initial report.
Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
documenting the corrective actions that have been or will be taken for each violation, and any supporting
paperwork, by 4/8/2020.
Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an
appointment, please call (209)468-3420.
Documents provided: Training Record template (Unidocs form UN-074), Hazardous Waste Labeling Minimum
Requirements, List of Free CUPA Training Classes, Return to Compliance Certification &Worksheet
FA0024355 PRO542389 SCO01 11/20/2019
EHD 22-01 Rev.9/20/2019 Page 15 of 16 Small Quantity Hazardous Waste Generator OIR
1868 E.
