Observation:A complete modified contingency plan was not posted at <br /> the facility. <br /> Banfield Response:As outlined in Banfield's Waste Management Plan, all Banfield hospitals are required to maintain a <br /> Contingency Plan, consisting of identifying emergency coordinators and post required information near the phone.The <br /> name and emergency phone numbers of the emergency coordinators(Director of Veterinary Quality& Field Director), <br /> location of emergency/spill equipment, and the local fire department number/911 have been posted in the hazardous <br /> waste storage area for this hospital.A photograph of the posting is provided in Attachment B (Photo 1). <br /> Violation#105: No emergency coordinator.
