S0 JOA U 1 Environmental Health Department <br /> U ; i y I AMENDED <br /> 3/9/2020 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Banfield Pet Hospital#1188 1 10520 Trinity Pkwy, Stockton November 20, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 202 HSC 25189.5(a)Disposed or caused disposal of HW at an unauthorized point. <br /> OBSERVATION:When asked by Denise Farias, Shift Leader, how the facility's tono batteries(used in equipment <br /> to test for eye glaucoma)are disposed of, Gabby Holdsworth, Registered Veterinary Technician, stated she <br /> disposes of the batteries into the trash like she does at home.According to the Banfield Pet Hospital Waste <br /> Management Plan, all batteries are to be managed as universal waste (see photos IMG 9552, 9577). <br /> REGULATION GUIDANCE: The disposal of any hazardous waste, or the causing thereof, is prohibited when the <br /> disposal is at a facility which does not have a permit from the department issued pursuant to this chapter, or at any <br /> point which is not authorized according to this chapter. Hazardous wastes shall be disposed of only by <br /> transportation to a permitted hazardous waste treatment, storage, and disposal facility(TSDF)or other authorized <br /> point. <br /> CORRECTIVE ACTION: Immediately cease disposal of batteries into the trash. Provide a corrective action <br /> statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class I violation. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> OBSERVATION:An open cardboard box, marked with the words"Expired Drugs,"and storing expired <br /> pharmaceuticals, according to Denise,was observed near the facility entrance door, on a high shelf, and directly on <br /> top of the white container labeled as Hazardous Waste Expired Rx Waste. Denise retrieved the cardboard box from <br /> the shelf for further observation (IMG 9531).According to the Banfield Pet Hospital Waste Management Plan, <br /> expired non-controlled pharmaceuticals, and hazardous expired controlled pharmaceuticals(included on the list in <br /> Appendix A), are to be managed as hazardous waste(see photos IMG 9567-70). <br /> REGULATION GUIDANCE:A container holding hazardous waste shall always be closed during transfer and <br /> storage, except when it is necessary to add or remove waste. <br /> CORRECTIVE ACTION: Immediately close these containers and ensure all hazardous waste containers are <br /> closed when not adding or removing waste.
