SAN J 0 A Q U 1 N Environmental Health Department <br /> U ; i y 1 Iii AMENDED <br /> 3/9/2020 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Banfield Pet Hospital#1164 2477 Naglee Rd,Tracy December 05, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> CORRECTIVE ACTION: Immediately dispose of the hazardous waste stored on site greater than 180 days in <br /> compliance with Title 22 California Code of Regulations. Provide a corrective action statement and a copy of the <br /> disposal records to the San Joaquin County Environmental Health Department(EHD)within 30 days. <br /> This is a Class II violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br /> were: <br /> 1.A white 1.25-gallon container labeled as DEA Controlled Substance Hazardous Waste and storing waste liquid <br /> controlled pharmaceuticals poured into kitty litter, according to Matthew Salaiz, Practice Manager, located in a <br /> cabinet under a sink,was missing the physical state on the hazardous waste label (see photos IMG 9681-8). <br /> 2.A white 1.25-gallon container labeled as and storing Hazardous Waste Formalin with Tissue, located in a <br /> cabinet under a sink,was missing the physical state on the hazardous waste label (IMG 9689-92). <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br /> action statement and supporting documentation to the San Joaquin County Environmental Health Department <br /> (EHD)within 30 days. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> -The facility hazardous waste generator has been changed back to Small Quantity generator status from RCRA <br /> Large Quantity generator status, since the business operator provided a written statement to the EHD, as well as <br /> a manifest correction letter to the DTSC, explaining the facility's epinephrine waste was mistakenly listed as <br /> P042 (a RCRA acute hazardous waste). The operator clarified that the epinephrine waste consists of an <br /> epinephrine salt, which is not included within the scope of the P042 listing, according to the EPA. Moving <br /> forward, the operator is to ensure the correct waste codes are listed on the facility's hazardous waste manifests. <br /> -The inspection report has been amended by changing the violations to reflect the facility's Small Quantity <br /> Generator status, including violation 108. <br /> -According to the Banfield Pet Hospital Waste Management Plan, Banfield disposes of all non-controlled <br /> pharmaceutical waste, and only the controlled pharmaceutical waste listed on Appendix A of the plan, as <br /> hazardous waste.
