Receiva bey sju/�'D(E7) on L012020 <br /> PF <br /> Barfield- <br /> PET HOSPITAL <br /> 18101 SE 61"Way, Vancouver WA 98683 • phone (360) 784-5000 • fax (360) 784-6000 <br /> April 8, 2020 <br /> VIA ELECTRONIC MAIL <br /> Ms. Elianna Florido <br /> San Joaquin County Environmental Health Department <br /> Certified Unified Program Agency <br /> 1868 East Hazelton Avenue <br /> Stockton, California 95205 <br /> RE: Response to Inspection Report Amended—9 March 2020 <br /> Banfield Pet Hospital#1164 <br /> CERS ID: 10643449 <br /> Dear Ms. Florido, <br /> On behalf of Medical Management International, Inc., d/b/a Banfield Pet Hospital (Banfield) I am writing to provide the <br /> response to your 9 March 2020 Amended Inspection Report issued by the San Joaquin County Environmental Health <br /> Department for the inspection conducted at Banfield Pet Hospital #1164 on 5 December 2019. <br /> Each violation is addressed below and the signed Return to Compliance Certification form in included in Attachment A. <br /> Violation#103:According to Matthew Salaiz, Practice Manager,the facility has been informed that the liquid waste <br /> from the VetScan HM 5 Hematology Reagent Pack equipment, observed in a white plastic container at the time of <br /> inspection may be disposed of as non-hazardous waste down the sink. Matthew confirmed that the facility pours this <br /> liquid waste down the sink once the container is full. Matthew did not have hazardous waste determination records that <br /> support the current handling and disposal of the waste at the time of inspection. <br /> Banfield Response: Banfield's HSE department retains the waste determinations for all hospitals.The manufacturer of <br /> the equipment and reagents provides a Certification of System Waste Composition (Attachment B) on its website. Using <br /> this analysis, Banfield determined that the waste does not meet the definition of a RCRA or non-RCRA hazardous waste. <br /> The waste determination and manufacturer-provided analytical are included in Attachment B. <br /> Violation#104: No modified contingency plan. Observation:A complete modified contingency plan was not posted at <br /> the facility. <br /> Banfield Response:As outlined in Banfield's Waste Management Plan, all Banfield hospitals are required to maintain a <br /> Contingency Plan, consisting of identifying emergency coordinators and post required information near the phone.The <br /> name and emergency phone numbers of the emergency coordinator(Field Director), location of emergency/spill <br /> equipment, and the local fire department number/911 have been posted in the hazardous waste storage area for this <br /> hospital.A photograph of the posting is provided in Attachment C(Photo 1). <br />
