1.0 INTRODUCTION <br /> On behalf of Phillips 66 Company (P66) and Atlantic Richfield Company (ARC), a BP Affiliated <br /> Company, Cardno ATC (ATC) is submitting this Monitoring Well Destruction Report (Report) to <br /> San Joaquin County Environmental Health Department (SJCEHD) for 76 (Former BP) Service <br /> Station No. 11194 (site). The Report was prepared in response to the No Further Action <br /> Required Concurrence letter, dated September 15, 2011, from the California Regional Water <br /> Quality Control Board—Central Valley Region (Appendix A). <br /> 2.0 SITE LOCATION <br /> The site is currently a closed 76-brand gasoline service station located at the northwest comer <br /> of the intersection of North Tracy Boulevard and Grant Line Road in Tracy, California (Figures 1 <br /> and 2). British Petroleum (BP) purchased the site from Mobil Oil in 1989 and operated at the site <br /> until ownership was transferred to Tosco Corporation (Tosco, now Phillips 66) in 1994. The <br /> current station configuration consists of a vacant station building with two auto service bays, two <br /> dispenser islands, and four double-walled fiberglass underground storage tanks (USTs); one <br /> 12,000-gallon capacity, two 10,000-gallon capacity, and one 1,000-gallon capacity. <br /> 3.0 SCOPE OF WORK <br /> The scope of work consisted of properly destroying eight onsite groundwater monitoring wells <br /> and seven offsite groundwater monitoring wells in compliance with State and County guidelines. <br /> Well destruction activities were conducted on September 6 and 7, 2012, September 10 and 11, <br /> 2012, and January 14 and 16, 2013. Of the 15 monitoring wells that were destroyed, 11 <br /> monitoring wells were destroyed by pressure grouting and the remaining four monitoring wells <br /> were over-drilled to total depth utilizing hollow-stem augers followed by grouting. <br /> 3.1 Site Health and Safety Plan <br /> As required by the Occupational Safety and Health Administration (OSHA) Standard <br /> "Hazardous Waste Operations and Emergency Response" guidelines (29 CFR 1910.120), and <br /> by California Occupational Safety and Health Administration (Cal-OSHA) "Hazardous Waste <br /> Operations and Emergency Response" guidelines (CCR Title 8, Section 5192), ATC prepared a <br /> Site-Specific Health and Safety Plan (HASP) prior to the commencement of fieldwork. The Site- <br /> Specific HASP was reviewed by field staff and contractors before beginning field operations at <br /> the site. <br /> 3.2 Permitting <br /> ATC obtained a well destruction permit from the San Joaquin County EHD prior to beginning <br /> work. Well destruction activities were observed and approved by Mr. Johnny Yoakum of the <br /> EHD.
