The <br /> monitoring wells must be destroyed by a C-57 licensed contractor under permit <br /> and inspection of PHSIEHD. Please submit a permit application with a $60.00 <br /> permit application fee. Provide PHSIEHD notification 48 hours in advance of the <br /> proposed monitoring well destruction so inspection of the work can be <br /> scheduled.
