SAN J OAQU I N Environmental Health Department <br /> CC LJ N T Y------- <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> AMAZON.COM SERVICES INC-DCK1 2403 W LOUISE AVE, MANTECA February 04, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> rO3 <br /> Remarks <br /> CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION:A fire extinguisher located on a pole in the southeast corner of the building had an indicator <br /> pointed to recharge. <br /> REGULATION GUIDANCE:All facility communications or alarm systems,fire protection equipment,spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the fire extinguisher into proper working order by repair, replacement or testing. <br /> Provide a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br /> This is a Class II violation. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> OBSERVATION: The following open hazardous waste storage containers were observed on-site: <br /> -One approximately 15 gallon clear plastic tote of waste cleaners was observed in the hazardous waste storage <br /> area open. <br /> -One approximately 15 gallon clear plastic tote of waste medications and supplements was observed in the <br /> hazardous waste storage area open and overfull. <br /> -One approximately 15 gallon clear plastic tote of class 9-miscellaneous waste was observed in the hazardous <br /> waste storage area open. <br /> -One approximately 15 gallon clear plastic tote of waste soaps&detergents was observed in the hazardous waste <br /> storage area open. <br /> -One approximately 15 gallon clear plastic tote of waste oxidizers was observed in the hazardous waste storage <br /> area open. <br /> -According to Alba Recinos(Safety Specialist), and as observed on the facility's uniform hazardous waste <br /> manifests, the waste listed above is sorted by and hauled off-site by Stericycle as hazardous waste <br /> REGULATION GUIDANCE:A container holding hazardous waste shall always be closed during transfer and <br /> storage, except when it is necessary to add or remove waste. <br /> CORRECTIVE ACTION: Immediately close these containers and ensure all hazardous waste containers are closed <br /> when not adding or removing waste.
