PHS/EHD must,be.consulted with:regard.:tcs::
the selection of soil sarrtpies whith will-undergcs laboratory
-analysis.,
The work plan also indicated that four groundwater monitoring wells would be installed as outlined in the
"Site Contamination Work Plan"dated February 1995. The Work Plan which was approved includedthe
drilling and sampling of four soil bor1rlgs, one of which was proposed for conversion into a monitoring..
well. PHSIEHD suggests that one of the three wells that are located in the supposed dawn gradient
direction be placed within 10 feet of the tank pit, unless the probe results suggest the cast effectiveness
of an interim remedial activity such as excavation. if excavation is not deemed possible as an interim
remedial measure, the referenced well can be used to evaluate remedial alternatives
PHS/EHO issued permit#12599 for the drilling<of four soil borings. PHS/EHD approves the expanded
scope of work which will be included under this:permit. Please note;that the sail probes shall be grouted
using a tremmie pipe. PHS/EHD staff must be contacted directly when field work is scheduled with the
driller and at least two working days prior to initiating the field work to confirm PHS/EHD staff s
schedule.
The report of the investigation's results should summarize all analytical results obtained and include
recommendations for the next phase of activity.
If you have any questions or wish to schedule held work, please contact Mary Meays at((249)468-0337.
Donna Heran, REHS, Director
Environmental Health Division
Mary Meays;Senior REH . tvlargar/ag!rio,"REH&
$IMe
Mitigation.lJn.it Supervisor
cc th1Z abe:.th Thayer;:Gentral Valley Regional Water Quality Control Board
Cc:
Bob:Marty;Advanced GeoEnvironmental'
14:.Divisian of sort Joacluiti Cotmty Health Care'Servkes
