SANJ O A Q U I N Environmental Health Department <br /> -C CSU N 'I 'Y <br /> Complaint Record Daily Time and Activity Summary from 5/26/2013 to <br /> 5/26/2020 for C00051960 located at <br /> 1900 ROOSEVELT AVE <br /> Activily Date Recorded by <br /> 05/15/2020 Received page from Genesis Martinez (Office Assistant). PG&E spill at 1900 PE 2546 BAKER <br /> 72 min Roosevelt Avenue in Escalon. Approximately 1 gallon of non-PCB mineral oil was SC 544 <br /> released. <br /> Called Celia Barajas of PG&E at 925-519-4750 and confirmed information in <br /> complaint. Celia Barajas said that the release happened the evening before and <br /> PSC already did the clean-up of the concrete and walls of the building. Clean-up <br /> of the roof will occur on Tuesday, 5/19/2020 when they get a lift. No storm drains <br /> or waterways were impacted by the release. Release was caused by equipment <br /> failure of an overhead transformer. One 55 gallon drum of waste was generated <br /> from the clean-up of the walls and concrete. <br /> Headed out to the site to verify the clean-up. Arrived on site and let business <br /> know I was there. Release was from a pole mounted transformer located next to <br /> the southeast side of the office. Clean-up of the walls and ground were complete. <br /> No oil was observed on the concrete or building walls. No storm drains were <br /> observed nearby. Wrote notice to abate and emailed it to Celia Barajas at <br /> CPM3@pge.com. Let her know to provide final disposal records and PCB <br /> analytical for the mineral oil. Also let her know that it looks like it will be raining on <br /> Sunday and Monday so the clean-up of the roof needs to done before that or the <br /> oil needs to be covered. Asked her to let EHD know when the clean-up will take <br /> place or send a photo of the covered roof. <br /> 05/18/2020 Called Celia Barajas of PG&E at 925-519-4750 to confirm that clean-up was still PE 2546 BAKER <br /> 120 min taking place tomorrow 5/19/2020. She said that with the rain that she decided to SC 544 <br /> have a PG&E crew clean the roof on Saturday instead of covering the roof for the <br /> rain. She said she would let me know how much waste was generated from the <br /> clean-up and send me a copy of the shipping paper. <br /> Went to the site to verify the clean-up of the roof. Notified business I was on site. <br /> It was raining at the time of the inspection. Did not observe any oil coming off of <br /> roof or sheen. Could not see top of roof even from a distance. Discussed clean <br /> up with owner Arlene Francis. She said that they cleaned everything and took quit <br /> a long time. Arlene Francis said that most of the oil went on the ground and not <br /> the roof. She also said that PG&E took a pallet of slate that oil got on and was <br /> waiting for someone to call her back on it. Let her know I would email her the <br /> contact info for the PG&E person when I returned to the office. Left site. <br /> Completed emergency response record and site map. Put copy of Notice to Abate <br /> on shelf to be mailed to property owner regular mail. <br /> Completed prop 65. Gave prop 65 to Elena Manzo (Lead Sr. REHS) for review. <br /> Completed photo log. Verified GIS in EC. <br /> Let Celia Barajas know to provide a photo of the roof after clean-up was <br /> completed for verification. <br /> 05/19/2020 Mailed Waste Discharge/Spill Response Notice to Abate dated 05/15/20 by PE 2546 MARTINEZ <br /> 0 min regular mail to: SC 177 <br /> William & Phelma Porter, <br /> 16453 E Milgeo Ave, <br /> Ripon CA 95366.
