SAN J O A Q U I N Environmental Health Department <br /> WASTE DISCHARGE/SPILL RESPONSE <br /> NOTICE TO ABATE <br /> Responsible Party: PG&E Date: May 15,2020 <br /> RP Mailing Address: PO BOX 7640 Record ID#: C00051960 <br /> SAN FRANCISCO, CA 94120- Program Element: 2546 <br /> Spill Location: 1900 ROOSEVELT AVE, ESCALON Release/Spill Response(excluding Joint Team) <br /> NATURE OF WASTE DISCHARGEMASTE RELEASE: <br /> Approximately 1 gallon of non-PCB mineral oil was released from a pole mounted transformer. <br /> OBSERVATIONS: <br /> Approximately 1 gallon of mineral oil was released onto the metal roof,walls and ground due to equipment failure of a pole <br /> mounted transformer. No waterways or storm drains were impacted. Clean-up is complete on the pavement and walls of <br /> the building. Clean-up of the roof will tentatively be completed on Tuesday, 5/19/2020 by PSC. One 55 gallon drum of oily <br /> debris was generated from the clean-up of the ground and building walls. <br /> PG&E to provide SJC EHD with PCB analytical results for the mineral oil. PG&E to notify SJC EHD when clean-up of the <br /> roof will be completed. PG&E to provide SJC EHD with final disposal records for the waste generated from the clean-up. <br /> VIOLATIONS: <br /> Discharge of any waste, including sewage <br /> (CA Health and Safety Code 5411) <br /> ° Discharge of waste or sewage to water or within 150 ft. of waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br /> Improper disposal of a hazardous waste <br /> (CA Health and Safety Code 25189.5) <br /> Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> El Other Violation <br /> ORRECTIVE ACTIONS/ORDER: <br /> Investigate and abate contamination under the oversite of the appropriate lead agency. <br /> Immediately <br /> Submit copies of hazardous waste manifests/disposal records to the EHD. <br /> Within 30 days. <br /> • Other Correction <br /> Submit copies of PCB analytical within 30 days. <br /> EHD 25-01 Rev.08/17/17 Page 1 of 2 WASTE DISCHARGE/SPILL RESPONSE NOTICE TO ABATE <br /> 1868 E.
