Within 15 days of any emergency incident which requires activation of the Contingency Plan, a <br />written report of the incident must be submitted to the local agency, describing the incident. <br />Example on page 8. <br />SECTION V: <br />HAZARDOUS WASTE/EMERGENCY TRAINING: <br />All employees handling, creating or having anything to do with management of hazardous waste <br />will be trained within 6 months of employment and annually thereafter by the Hazardous Waste <br />Coordinator. The Hazardous Waste Coordinator will have annual training from an outside <br />qualified agency on hazardous waste management procedures. <br />Training will include emergency procedures for spills, ground/water contamination, fires, use <br />and care of fire extinguishers, notification to the proper authorities and proper disposal of <br />universal waste. All employees involved will also be instructed on how to lessen or eliminate the <br />generation of waste. <br />DOT Hazmat training will be obtained, at least every three years, for all employees involved in <br />shipping/receiving hazardous material, specifically, all employees of the Shipping/Receiving <br />Department, as well as designated employees that sign hazardous waste manifest forms, land <br />disposal notification forms upon vendors removing waste from the facility. <br />7 <br />
