12 <br /> SITE SAFETY AND HEALTH PLAN <br /> Project Summary <br /> Brown and Caldwell will supervise the drilling and sampling of three boreholes with <br /> monitoring wells installed in each of the boreholes at the site. The purpose of the samples <br /> is to determine the extent of hydrocarbon contamination in the soil and/or groundwater <br /> beneath the site. <br /> Brown and Caldwell will provide one geologist at the site to supervise the drilling and <br /> sampling procedures. Duration for the activity at the site will be approximately 32 hours. <br /> i <br /> Key Personnel and Responsibilities <br /> The project manager for borehole drilling and soil sampling activities will be <br /> Mr. Larry Phillips. The project manager is responsible for establishing and enforcing <br /> safety rules and procedures for the project, and ensuring that project team members have <br /> received the required training prior to field activities. The project manager will ensure that <br /> each project team member receives and reads a copy of this site safety and health plan <br /> (SSHP) and signs the SSHP acknowledgement form. <br /> The Corporate Health and Safety Officer is Ms. Pam Bare, CIH. Mr. Steve Misner is <br /> the project geologist and the on-site sampler and site safety officer (SSO). The SSO has <br /> completed 40 hours of comprehensive safety training, meeting all requirements of Title 29, i <br /> Code of Federal Regulations, Part 1910.120 (29CFR 1910.120) for hazardous materials <br /> operations. The SSO has the authority to correct health and safety problems on site. <br /> The SSO has on site responsibility for ensuring that Brown and Caldwell on-site <br /> personnel comply with the requirements of the SSHP. The SSO responsibilities include: <br /> 1. Updating equipment or procedures used on site in response to changing situations. <br /> 2. Inspecting all personal protective equipment prior to any on-site use. <br /> 3. Performing and evaluating the effectiveness of decontamination procedures for <br /> personnel, protective equipment; and sampling equipment and containers. <br /> a 4. Monitoring for symptoms of exposure or stress. <br /> i <br /> 5. Ensuring availability of adequate drinking water supply.
