<br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System
<br /> (CERS)for 2018, 2019, and 2020. As of January 1, 2013, all businesses were required to submit all business plans
<br /> electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information,
<br /> and submit to the EHD for approval.
<br /> This is a repeat violation, Class 11.
<br /> 3 HSC 25507 Failed to establish and implement a hazardous materials business plan.
<br /> A Hazardous Materials Business Plan has not been established or implemented for 2018, 2019, and 2020. A
<br /> Hazardous Materials Business Plan shall be established and implemented for emergency response to a release or
<br /> threatened release of a hazardous material when storing hazardous materials at or above the thresholds quantities
<br /> of 55 gallons for materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard
<br /> temperature and pressure. Immediately log into the California Environmental Reporting System (CERS) at
<br /> http://cers.calepa.ca.gov/, enter the required information, and submit for review by the EHD.
<br /> This is a repeat violation, Class 11.
<br /> 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page.
<br /> The Business Activities and Business Owner/Operator Identification forms have not been submitted for 2018, 2019,
<br /> and 2020. This information must be current and submitted as part of the business plan. Immediately log into the
<br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated
<br /> information, and submit to the EHD for approval.
<br /> This is a repeat violation, Class 11.
<br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site.
<br /> 56 gallons of propane, greater than 200 cubic feet of compressed oxygen, and greater than 200 cubic of acetylene
<br /> (actual amount of compressed and acetylene was not available due to missing volume markings)were observed on
<br /> site and have not been reported for 2018, 2019, and 2020. Any material that meets or exceeds the reportable
<br /> quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting
<br /> System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to
<br /> the EHD for approval.
<br /> This is a repeat violation, Class 11.
<br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content.
<br /> A site map was not completed or submitted as part of the business plan for 2018, 2019, and 2020. A site map shall
<br /> contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit
<br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and
<br /> emergency response equipment. If a site map element is not applicable for your facility then list it on the map and
<br /> label as "NA". Immediately log into the California Environmental Reporting System (CERS) at
<br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
<br /> This is a repeat violation, Class 11.
