The following is an itemized list of hazardous waste violations that have not been <br /> addressed for ALL STAR HEAVY HAUL & TOW as of Junel 120201 <br /> Open violations from October 15 , 2019 inspection <br /> Violation #106 - Failed to train employees on waste handling and emergency procedures . <br /> OBSERVATION At the time of inspection , it could not be demonstrated that employees who handle hazardous <br /> waste were properly trained . <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with proper <br /> waste handling and emergency procedures , relevant to their responsibilities during normal facility operations and <br /> emergencies . <br /> CORRECTIVE ACTION: Immediately train the employees . Provide proof of training within 30 days to the San <br /> Joaquin County Environmental Health Department ( EHD ) for employees whose responsibilities include hazardous <br /> waste . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) . <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #113 - Failed to keep copies of consolidated manifesting receipts for three years . <br /> OBSERVATION Copies of consolidated manifest receipts for hazardous wastes including used oil , and household <br /> hazardous waste were not found on site during the inspection <br /> REGULATION GUIDANCE: The generator shall retain each consolidated manifest receipt for at least three years . <br /> This period of retention is extended automatically during the course of any unresolved enforcement action regarding <br /> the regulated activity or as requested by the department or a certified unified program agency. <br /> CORRECTIVE ACTION : Immediately locate a copy of all consolidated manifest receipts from the past three years <br /> and maintain them on site . Provide a copy of the consolidated manifest receipts and a corrective action statement <br /> to the San Joaquin County Environmental Health Department. <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) . <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Page 1 of 2 <br />
