The following information is still needed in order to <br /> close the remaining violations: <br /> 1. Violation 101: Submit a written statement with your State ID number, once you obtain the ID number. <br /> 2. Violation 102 and 106: <br /> a. Submit a written statement/document that answers the following questions regarding the facility's <br /> hazardous waste program: <br /> i. How and where does the facility generate hazardous waste? <br /> ii. What are the types of hazardous waste the facility generates? <br /> iii. How do facility employees identify, store, and label the different types of hazardous waste, <br /> while ensuring that incompatible wastes do not commingle? <br /> iv. How often do employees inspect facility areas where hazardous waste is generated to ensure <br /> unidentified hazardous waste is not being left behind? <br /> v. How often do employees inspect the facility's hazardous waste storage area for leaking and <br /> deteriorated hazardous waste storage containers? <br /> vi. How does the facility dispose of the hazardous waste it generates? <br /> vii. How will the facility maintain 3 years of hazardous waste manifests (disposal records) on-site at <br /> the facility? <br /> b. Submit a statement explaining how the two 1-gallon containers of Diversey Oxivir Tb (General Virucide, <br /> Bactericide,Tuberculocide, Fungicide, Sanitizer) and two 1-quart containers of Sustainable Earth by <br /> Staples Carpet Spotter, which were observed on plywood shelving in the facility's back storage/receiving <br /> area, were managed. If these items were disposed in the trash, explain how it was determined that <br /> these items were not hazardous waste. If these items were managed as hazardous waste, <br /> explain/provide documentation that shows how these items were stored and labeled. <br /> 3.
