Per my phone conversation with Mike earlier today, attached is the completed inspection report and inspection <br /> photos from the hazardous waste inspection conducted at Falling Prices in Tracy on 11/21/2019. <br /> Also attached is the Modified Contingency Plan template, Hazardous Waste Labeling Minimum Requirements <br /> guidance sheet, and an updated List of Free CUPA Training Classes. These documents were provided to Philip <br /> Parra at the time of inspection. <br /> Complete and submit a copy of the attached Return to Compliance Certification form and worksheet to the San <br /> Joaquin Environmental Health Department(SJC EHD) with a statement documenting the corrective actions <br /> that have been or will be taken for each violation, and any supporting paperwork, by 1/2/2019. <br /> Please note that our policy at SJC EHD does not grant extensions for a business to remain out of compliance, <br /> and the facility may be billed if the required information is not received.
