Violations 101, 104, and 105: Submit a written statement documenting if your business will generate any <br /> hazardous waste on site, including used oil and used antifreeze, or not. If your business will generate any <br /> hazardous waste,your business will need to obtain a permanent state EPA I number, and post a complete <br /> modified contingency plan at your facility. <br /> 2. Violation 102:Submit a written statement, and disposal records,documenting how you managed the following <br /> wastes,which were observed at the time of inspection: <br /> a. An unlabeled bag of oily rags observed on top of the facility's used oil tank. <br /> b. A 5 gallon container of Premium Fibered Aluminum Roof Coating observed on top of the facility's used <br /> oil tank. <br /> c. An open container labeled ChemStation Heavy Duty Degreaser, containing either degreaser or used oil, <br /> was observed on the ground next to the facility's used oil tank. <br /> d. A blue container labeled Henry, containing what appeared to be a chemical sealant,was observed on <br /> top of the facility's used oil tank. <br /> 3. Violations 110 and 113: Submit a written statement documenting if your business generated hazardous waste, <br /> including used oil and used antifreeze, in the last three years. <br /> 4. Violation 301:Submit a written statement documenting how you cleaned the oil spills and staining that were <br /> found on top of and around the oil tank. Also, document how you managed the waste generated from the spill <br /> clean-up, and submit disposal records for the spill clean-up waste. <br /> 5. Violation 505:Submit a written statement documenting if and how you will continue to use the used oil tank. If <br /> the tank is not going to be reused to store used oil, and the tank is not cleaned,the tank must be disposed of as <br /> hazardous waste. If the tank is going to be recycled as scrap metal,the tank must be cleaned via triple rinsing. <br /> Submit a hazardous waste manifest for the disposal of the tank,or submit disposal records hazardous waste <br /> manifests for the rinse water and a receipt for the recycling of the tank. <br /> 6. Violation 601 and 605: Submit a hazardous waste manifest or consolidated manifest receipt for the disposal of <br /> the 55-gallon drum of waste antifreeze observed at the time of inspection. <br /> Submit this information immediately to avoid being billed by our department for non-compliance. <br /> Please contact me if you have any questions. <br /> t <br />
