The following is an itemized list of hazardous materials violations that have not been addressed for JAAS AUTO SERVICE as of July23, 2020. Open violations from October 112018 inspection Violation#5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. The Hazardous Materials Inventory forms for waste oil and waste antifreeze are not complete. The annual amount waste amount has not been completed. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #6 - Failed to complete and/or submit a site map with all required content. The map does not show storm and sewer drains, the new and waste antifreeze, emergency response equipment (spill control and fire extinguishers), and a legend to identify the codes and abbreviations used on the map. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened release. The emergency response procedures have not been submitted. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment - Evacuation plans and procedures, including immediate notice, for the site Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
