The following is an itemized list of hazardous materials violations that have not been <br /> addressed for JAAS AUTO SERVICE as of July23, 2020. <br /> Open violations from October 112018 inspection <br /> Violation#5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms for waste oil and waste antifreeze are not complete. The annual amount <br /> waste amount has not been completed. This information must be current and submitted as part of the business <br /> plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> enter the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not show storm and sewer drains, the new and waste antifreeze, emergency response equipment <br /> (spill control and fire extinguishers), and a legend to identify the codes and abbreviations used on the map. A site <br /> map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 3 <br />
