4. WELL DECOMMISSIONING <br /> Based on the SJCEHD's directive dated April 2, 2014, SOMA proposes to <br /> decommission MW-4 in accordance with California Well Standards. The existing <br /> well will be decommissioned by over drilling to the total well depth. Well casing, <br /> as well as all annular materials, will be removed and the boring will be tremie <br /> grouted using Type 1/II cement grout. Well box will be removed and well <br /> completed to grade with a neat cement and bentonite mixture. Well construction <br /> diagram of MW-4 is attached in Appendix B. <br /> 5. WASTE DISPOSAL AND REPORT PREPARATION <br /> Soil and well construction material generated from the well decommissioning <br /> activities will be temporarily stored on-site in DOT rated drums pending transport <br /> to an appropriate licensed disposal facility. Upon completion of field activities, <br /> SOMA will prepare a report documenting a description of completed field <br /> activities. The waste manifest will be included in SOMA's well decommissioning <br /> report. <br /> 6. SCHEDULE <br /> SOMA anticipates that the scope of work described in this workplan will be <br /> completed within 4 weeks, under normal conditions, from the date of obtaining <br /> necessary approvals, authorizations, and permits.
