IN REPLY REFER TO SITE CODE: 2454 <br /> 1444 Tillie Lewis Drive <br /> Stockton, CA <br /> San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) has reviewed the Problem Assessment Report dated <br /> May 30, 1991, and the Confirmation Sampling Report dated August 6, <br /> 1991, both submitted by WaterWork Corporation and has the following <br /> comments. <br /> Pursuant to Section 2652 (d) of the Underground Storage Tank <br /> Regulations (California Code of Regulations, Title 23, Subchapter <br /> 16) a quarterly monitoring report shall be submitted to Public <br /> Health Services, Environmental Health Division and Central Valley <br /> Regional Water Quality Control Board (CVRWQCB) until the <br /> investigation and cleanup of the site is deemed adequate. The <br /> report is to show the: <br /> --status of investigation and cleanup activities including <br /> the results of all investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for <br /> the completion of the investigation of the site and <br /> remediation; <br /> -method and location of disposal of the released <br /> hazardous substance and any contaminated soil, <br /> groundwater or surface water; <br /> -manifest required for transport of hazardous <br /> substances; <br /> -monitoring well data including groundwater elevation, <br /> pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if <br /> known, and changes in the above measurements over the <br /> last quarter. <br /> Please continue to submit Quarterly Monitoring Reports. <br /> A 1)jFisiun of son i(minin county Ilcahh Carc M•rnices <br /> i <br />
