The incorrect and missing information included the following from the inspection reports: <br /> -The designated operator failed to document all the alarms from the attached alarm history on the designated <br /> operator monthly inspection reports dated 02/7/2020 and 9/13/2019 . The missing alarms include: <br /> -L10 piping sump ) sensor out alarms on 01/30/2020 ,and 12/30/2019. <br /> L8(STP sump)fuel alarm on 08/16/2019 <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 302 HSC 25291(b) UST system incapable of detecting leak from primary into secondary containment (pre-Jul 2003). <br /> OBSERVATION: The jet fuel T4 annular sensor failed to activate an audible and visual alarm when tested and, <br /> therefore, the tank is not monitored. <br /> REGULATION GUIDANCE: The UST system shall be designed and constructed with a monitoring system capable <br /> of detecting leaks from primary containment into secondary containment. <br /> CORRECTIVE ACTION: The sensor was replaced and retested during the inspection. A"UST Retrofit Verification <br /> with Inspector Already Onsite"form has been completed and provided to the operator and contractor. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by March 25, 2020. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
