SAN J O A R N Environmental Health Department <br /> -:a U ;� i _# �V AMENDED <br /> 7/23/2020 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MORADA CHEVRON FAST N EASY#60* 10878 N HWY 99 , STOCKTON May 28, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 105 CCR 2632(d)(1), 2634(d)Failed to have an approved UST monitoring plan. <br /> OBSERVATION: The monitoring plan is not current and/or not approved by the EHD. <br /> -The monitoring plan for all tanks incorrectly stated pipeline integrity testing was being conducted annually. <br /> -UDC leak sensor model information was left blank on the monitoring plans for all tanks. <br /> -The monitoring plan incorrectly indicated no for training. <br /> REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting <br /> System (CERS). <br /> CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. <br /> This is a repeat violation, Class ll. <br /> 107 CCR 2632(d), 2641(h), 2711(a) Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> OBSERVATION:An accurate UST Monitoring Site Plan was not submitted. <br /> -Map does not clearly identify the underground storage tank piping. <br /> REGULATION GUIDANCE:A site plan must be submitted identifying the locations where monitoring will be <br /> performed. The site plan must show the general layout of tanks and piping and clearly identify locations of the <br /> following equipment, if installed: 1)monitoring system control panels; 2) in-tank liquid level probes (if used for leak <br /> detection); 3)devices monitoring tank annular spaces or vault;4)devices monitoring product piping; 5)devices <br /> monitoring fill piping; 6)devices monitoring vent piping; 7)devices monitoring vapor recovery piping; 8)devices <br /> monitoring vent/transition sumps; 9)devices monitoring under-dispenser containment; 10)line leak detectors; and <br /> 11)devices monitoring any other secondary containment areas. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation, Class ll. <br /> 111 CCR 2638, 26410),2715(f) Failed to test monitoring and leak detection equipment every 12 months. <br /> OBSERVATION: The annual monitoring system certification and leak detector testing were last performed on <br /> 4/11/2019 and were due by April 2020. Testing has not been completed and is one month past due. <br /> REGULATION GUIDANCE: These tests are required once every 12 months. <br /> CORRECTIVE ACTION: Immediately schedule these tests and provide 48 hours notification to the EHD.
