S0 JOA U 1 Environmental Health Department <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Mossdale Chevron 444 W MOSSDALE RD, LATHROP June 10, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 319 CCR 2635, 2637.1,2665, 2712 No spill bucket,failed to contain 5 gallons, no means to keep it empty, or failed to be <br /> tested. <br /> OBSERVATION: The 87 and 91 product direct bury spill containers failed to have a minimum capacity of five <br /> gallons. <br /> REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to repair or <br /> replace the spill container under permit and inspection of the EHD. <br /> This is a repeat violation, Class ll. <br /> 320 CCR 2635, 2637.2,2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> OBSERVATION: Overfill prevention inspection was conducted on 1/10/2019 and test results were submitted to EHD <br /> on 8/19/2019, 6 months late.The test report did not include tank charts and accurate calculations for the 87, 91, and <br /> diesel tanks.The percent capacity of the tanks at which the overfill prevention equipment activates appeared to be <br /> calculated linearly.The Industry Code of Engineering Standards (PEI 1200)was used for the inspection method <br /> rather than the manufacture guidelines. This test result is not accepted by EHD. <br /> REGULATION GUIDANCE:All UST systems must be equipped with an approved,functional overfill prevention <br /> device at all times during delivery operations. <br /> CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br /> repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the failed component can't be replaced immediately,there is a possibility that the 87, 91, and diesel system may <br /> be red tagged to prevent fuel inputs. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 7/9/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
