SAN - J O A Q U IN Environmental Health Department <br /> COUNTY— <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> WALGREENS#2680 1 15 W HARDING WAY, STOCKTON June 11, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br /> were: <br /> 1.A Corrosive Acidic Hazardous Waste container, storing two bottles of Lysol cleaner waste,was missing the <br /> name and address of the generator. <br /> 2.A Toxic Hazardous Waste container, labeled with an accumulation start date of 5/25/2020 and storing <br /> over-the-counter pharmaceutical waste,was missing the name and address of the generator, and was not labeled <br /> with the accurate composition(contents). <br /> 3.A Toxic Hazardous Waste container, labeled with an accumulation start date of 5/23/2020 and storing <br /> over-the-counter pharmaceutical and supplement waste,was missing the name and address of the generator, and <br /> was not labeled with the accurate composition(contents). <br /> 4.A black container with a yellow lid, located in the pharmacy, labeled with the words"for incineration only,"and <br /> storing a bag of loose pharmaceutical pills and a bag of manufacture-packaged pharmaceuticals,was missing the <br /> words"Hazardous Waste', name and address of generator, hazardous properties, physical state, composition <br /> (contents), and accumulation start date. <br /> 5.A black container with a yellow lid, located in the pharmacy, labeled with a list of"P-Listed Items,"and storing <br /> warfarin tablet bottles containing residual warfarin,the words"Hazardous Waste", name and address of generator, <br /> hazardous properties, physical state, and accumulation start date. Darren Wong, a pharmacist, labeled the <br /> container with the required information at the time of inspection. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br /> action statement and supporting documentation to the San Joaquin County Environmental Health Department <br /> (EHD)within 30 days. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 7/11/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
