The permit has been <br /> approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District <br /> for any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work . All new addenda supersede the previous ones . <br /> 3 . Provide written notification to ust (a), slgov . org or contact Shewit Woldearegay at (209 ) 468- <br /> 0216 to arrange the following appointments ( a 48- hour notice is required for inspection <br /> appointments ) . <br /> • Overfill prevention verification : Measurement of drop tubes to verify 95 % tank <br /> volume shut off. Verification of flapper functionality . Ensure that all pertinent tank <br /> charts and calculations are available for review at the time of inspection . <br /> • Spill bucket test : Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent) . Proper drainage must also be verified . <br /> • If concrete is broken near the tank , a secondary containment test must be <br /> performed on the 87 tank once concrete is poured in accordance with the <br /> manufacturer' s guidelines . <br /> Please be aware that the facility cannot receive fuel in the 87 tank prior to the EHD <br /> inspection and approval . <br /> 4 . Any individual performing the work of a service technician must meet the following <br /> qualifications : <br /> a . Possess or be employed by a person who possesses one of several permissible valid , <br /> current contractor' s licenses issued by the Contractors State License Board ( may be <br /> issued to the installation company) , or a Tank Tester' s license issued to the individual <br /> by the State Water Board , and <br /> 1868 E .
